What is Palliative Care?

Palliative Care is a medical approach that focuses on providing relief from the symptoms and stress of a life threatening illness. The goal is to improve quality of life for both the patient and the family.  Unlike hospice care, palliative care can start at the time of diagnosis of a serious illness and be provided along with curative treatment.

Do I need a referral?

We do accept referrals from physicians, hospitals, hospice companies, and health plans, etc. Although, a referral is NOT required unless you have an HMO plan.

Who is on my care team?

The initial visit will be completed by a physician or nurse practitioner. Your team will depend on your medical needs but may include all of the following disciplines.

  • Physical Therapy

  • Occupational Therapy

  • Dietician

  • Social Worker

Do I qualify for this program?

Resilient Healthcare's Palliative Care Program may be right for you if you have been diagnosed with a serious illness such as cancer, heart disease, chronic lung disease, kidney disease, Alzheimer’s, HIV/AIDS, amyotrophic lateral sclerosis (ALS), multiple sclerosis, COVID19, or Parkinson’s etc. and are experiencing pain, fatigue, weakness, or difficulty managing symptoms. Especially if you are considering a long term care or skilled nursing facility.

Will my insurance pay for this program?

We accept most insurances including Medicare and Medicaid. Benefits will vary depending on your policy.

Can I use Resilient HC after my joint replacement? 

Yes! For patient's who would like to return home sooner after surgery, Resilient is an alternative to inpatient rehab.
