Alexander Peralta, Jr., BS Pharm., MD, DABHPM, HMDC

Speaker, Author, Pain and Palliative Medicine Specialist 

Dr. Alexander Peralta has 30 years of educational experience in the fields of pharmacy, internal medicine, hospice and palliative medicine. He received  his BS in pharmacy at the University of Texas at Austin and holds an MD jointly from Centro de Estudios Xochicalco, Cuernavaca, Mexico and from New York Medical College. In 1982 he moved to Duncanville, Texas and practiced acute care and general internal medicine for several years.  He has served two hospital systems as Department Chair, Chief of Staff and as a trustee and was the Chief Medical Officer for Family Hospice, Inc. from 1992-1999.   


He has served as clinical faculty for UTHSCD/SWMS and UNTHSCFW/TCOM, was Deputy Director of the UNTHSCFW/TCOM Palliative Medicine Fellowship Program, and was certified and recertified as a Diplomate of Hospice and Palliative Medicine by the ABHPM and Hospice Medical Director Certified. 


Dr. Peralta has served on international, national and state boards focused on hospice and palliative care including Texas Partnership for End-of-Life Care, NHPCO’s Access and Diversity Advisory Council, Diversity Steering Committee and NHPCO’s National VA Advisory Council.  Dr. Peralta has presented at International, national, state and local symposia.  He has authored multiple publications. 

In 2004, he was selected to serve on the Texas Department of Health and Human Services’ Aged and Disability Committee by the Deputy Commissioner of TDHS.   The Texas Partnership named Dr. Peralta “A Champion in End-of-Life Care” in 2005.  In 2006, the Fort Worth Texas magazine’s selected him as Top 100 Doctors.  His memberships include the AMA, TMA, Dallas County Medical Society, American Academy of Hospice and Palliative Medicine, and Texas Academy of Palliative Medicine.  He is a Fourth Degree Knight in the Knights of Columbus at Holy Spirit Catholic Church.   He is a Past President of the Rotary Club of Duncanville and has severed as Youth Director, Grants Chairman and Foundation Chair. 


He is a recipient of the Serve Above Self and Rotarian of the Year Awards. He is the Chief Operating Officer for Peralta Enterprises, President of Palliative Medicine Consulting Services, Emeritus Founding Board Member of Resilient Healthcare and Director of Hospice and Palliative Care for Centric Healthcare.  

Dr. Peralta has helped us mold our vision and continues on staff as a treating physician and consultant.

Internal Medicine


Dr. Sarang Desai  
Orthopedic Surgeon

Dr. Desai received his medical degree from the University of North Texas Health Science Center in Fort Worth and went on to complete an orthopedic surgery residency at St. James Hospital in Chicago. Following residency, Dr. Desai was selected for one of only two coveted AO foot and ankle fellowships in the country at Michigan International Foot and Ankle Center in Detroit. Dr. Desai is the only Orthopedic Surgeon Fellowship Trained in Foot and Ankle surgery practicing in Allen extending north to Oklahoma, and is the only orthopedic surgeon in McKinney focused 100% on foot and ankle injuries and disorders. He has served on the American Orthopaedic Foot and Ankle Society Post-Graduate, the Education Committee, and has participated as a reviewer for the Journal of Surgical Orthopaedic. He believes strongly in both physician and patient education and lectures nationally and internationally, instructing other surgeons and medical professionals. 

Dr. Desai oversees our orthopedic department and is committed to improving post operative outcomes for all orthopedic patients. 



Nejat Tewolde NP
Nurse Practitioner

Nejat Tewolde, APNP, MSN, BSN is a broad certified Family Nurse Practitioner. She completed Bachelor of Science in Nursing at Grand Canyon University. After spending more than eight years at the bedside caring for oncology and hematology patients, Nejat went to receive her Maters in Nurse Practicing at University of Texas of Arlington. Her work experience involved in treating oncology patients, palliative care and hospice in in-patient setting. Nejat has also worked in primary care as a Nurse Practitioner practicing in preventative medicine across the lifespan. She also has a certification in Botox injections that she practices on her spare time.
Nejat is strong believer in a the matter of quality of life, individuals may have different measurement when it comes to quality of life and as healthcare professionals it is our responsibility to meet their goals. 

Jeanette Cabarello - Bell
Adminisitrative Director

Jeanette was in the legal field for 24 years before switching gears and focusing her management and administration skills on healthcare. Jeanette started out with PARS staffing agency, managing all day to day operations and handling growth of the company with ease. In 2019, Jeanette moved into the Administrative Director role for Resilient Healthcare, overseeing daily operations for the physicians and nurse practitioners in the field. Her legal background has equipped for the ever changing healthcare industry. She has a strong passion for our patients, and is committed and dedicated in assisting our clinical team to make sure all of the patient’s needs are met from start of care to the end of treatment.
